저희 큰 아이는 올해 6살인 사내아이입니다어려서부터 감기에 걸리면 숨쉬기가 힘듭니다.

평상시에도 약간의 코골이가 있구요감기에 걸렸을 때는 특히밤에 잠을 잘 때는 숨 쉬는 소리가 어찌나 힘겨운지 그 리듬(?)에 같이 숨차할 때가 많습니다해서 종합병원에 가서 x선을 찍어보았습니다의사선생님께서는 아데노이드가 비대해서 수술로 제거(?)해 주어야 할 것 같다는 말씀을 하십니다소아청소년과 선생님께서는 성장하면서 작아질 수도 있으니 그냥 지켜보자고 하시구요전 환절기(가을겨울)가 두렵습니다가끔씩 멀미가 난다고 하는 걸 보면 두통도 심한 것 같구요이비인후과에 가면 코가 자꾸 뒤로 넘어간다고 축농증도 있다고 하시고이제 겨우 몸무게가 19kg입니다숨쉬기 힘들어 가는 걸 보면 당장이라도 수술해 주고 싶은 맘 뿐입니다지난번 겨울엔 편도에 박테리아 염증이 심해서 거의 먹지도 못하고 온 입술에 물집까지 번지고 일주일내 탈수상태까지 간 적도 있거든요.

어려서부터 감기에 걸리면 숨쉬기가 힘듭니다

감기는 비강인두 등 상기도의 점막층 감염으로 생기는 바이러스 상기도 감염 병입니다.

감기에 걸리면 코가 막혀서 숨쉬기가 좀 곤란할 수 있습니다그렇지만 그 정도로 숨쉬기가 힘들게 감기로 코가 막히지 않는 것이 보통입니다.

감기와 알레르기 비염감기와 기관지 천식특히 야간 수면 유발성 기관지 천식또는 감기알레르기 비염기관지 천식이 함께 있을 수 있습니다그럴 때 호흡 곤란이 심하게 생길 수 있습니다.

거기다가 아데노이드 비대로 호흡곤란이 더 심해 질 수 있읍니다.

평상시에도 약간 코골이가 있구요

A.

코골이가 있는 대부분의 소아 청소년들에게는 알레르기 비염이 있고 그로 인해 비강 점막층이 붓을 수 있습니다그 때문에 비강 기도 부분 속이 더 좁아질 수 있습니다.

아데노이드와 편도가 비대 되고 그로 인해서 인두 강과 비강 사이의 기도의 통로가 좁아지고 편도도 커져서 인두강 기도 부분이 더 좁아지고 결국 상기도 통로 전체가 더 좁아질 수 있습니다.

아데노이드가 많이 비대 되고 편도 비대기 있으면 비강 기도 부분을 통해서 공기를 하기도 속으로 흡인하는 데 어려움이 생기고 결국 호흡곤란이 생길 수 있습니다.

때로는 수면 중 차단성 무호흡증이 생길 수 있습니다.

부비동비강아데노이드 CT 스캔 검사로 아데노이드가 얼마나 커있는지 알 수 있고 부비동염이 있는지 없는지 알 수 있습니다.

아데노이드가 커지고 알레르기 비염이 있는 소아 청소년들의 거의가 부비동염이 함께 있는 경우가 많습니다.

알레르기 비염을 적극적으로 치료하고부비동 염은 적절한 항생제로 2~6주간 치료한 이후에도아데노이드가 계속 비대 되서 숨쉬기가 곤란하든지 차단성 수면 무호흡증이 계속 있으면 이비인후과 전문의의 진료를 받고 그분이 동의하시면 아데노이드 절제 수술을 해 주십시오.

감기에 걸렸을 때는 특히밤에 잠을 잘 때는 숨 쉬는 소리가 어찌나 힘겨운지 그 리듬(?)에 같이 숨차할 때가 많습니다. 

A.

위에서 설명 드린 바와 같이 감기 알레르기 비염 아데노이드 비대 및또는 편도 비대나 기관지 천식으로 그런 증상이 생기는 것 같습니다.

천식은 아토피성 피부염과 알레르기 비염이 있는 소아 청소년들에게는 거의 80~90% 생길 수 있다고 생각하시면 거의 틀림없습니다.

소아청소년과에서 진료 받고 알부테롤(Albuterol) 등 기관지 확장제로 치료하고,

프레드니손제코르티코스테로이드제 등 기관지 소염제로 기관지 천식을 며칠 동안 치료하면 호흡곤란의 중증도가 아주 좋아질 것입니다.

비강 내에 뿌리는 알레르기 비염 치료용 코르티코스테로이드제 코 분무와 2세대 경구용 항히스타민제로 알레르기 비염을 치료하면 치료 효과가 많이 날 것입니다.

의사선생님께서는 아데노이드가 비대해서 수술로 제거(?)해 주어야 할 것 같다는 말씀을 하십니다소아청소년과 선생님께서는 성장하면서 작아질 수도 있으니 그냥 지켜보자고 하시구요전 환절기(가을겨울)가 두렵습니다.

이런 건강 문제로 고생하는 소아 청소년들이 많이 있습니다.

특히 환절기에 감기와 기관지 천식 등의 호흡기 질환이 더 잘 생기기 때문입니다.

비강 속에 장기간(몇 주 내지 몇 달 동안뿌려 치료하는 크로몰린 소듐(Cromolyn Sodium) 소염제로 치료할 수 있습니다부작용이 거의 없고 알레르기 비염을 예방하는데 좋은 효과가 있습니다크로몰린은 미국에서는 의사의 처방 없이 약국 OTC에서 살 수 있는 약이지만 알레르기 비염 치료에 적어도 한 번 써볼 수 있는 약입니다.

그 외 비강 속에 뿌려 알레르기 비염을 치료하는 코르티코스테로이드 분무제도 있고 클라리틴(Claritin) 등 경구용 2세대 항히스타민제로 알레르기 비염을 치료합니다(표 참조).

가끔씩 멀미가 난다고 하는 걸 보면 두통도 심한 것 같구요이비인후과에 가면 코가 자꾸 뒤로 넘어간다고 축농증도 있다고 하시고..

A.

아토피성 체질이고 자녀와 같이 알레르기 비염 등이 있는 소아 청소년들의 얼굴이 아데노이드 얼굴형(아데노이드 안모)일 수 있고 부정교합이 생길 수 있고 치열이 고르지 않을 수 있고 알레르기 샤이너 등 징후가 생길 수 있습니다.

여러 가지 이유로 멀미도 잘 하고 모든 신체 계통이 예민할 수 있습니다.

비강 점막층이 알레르기 비염으로 붓고 그로 인해서 부비동과 비강을 연결하는 자연 통로가 쉽게 막힐 수 있습니다.

따라서 부비동염이 더 잘 생기게 됩니다.

콧물이 후비강으로 연결되는 인두강 속으로 넘어가서 부비동염이 생기는 것은 아닙니다.

Q.

이제 겨우 몸무게가 19kg입니다숨 쉬기 힘들어 하는 걸 보면 당장이라도 수술해 주고 싶은 맘뿐입니다.

A.

체중은 50퍼센타일 정도 됩니다.

상기도가 아데노이드 비대 등으로 막히면 숨 쉬기도 곤란하고 밤에 잠을 제대로 잘 수가 없기 때문에 낮에 많이 피로할 것이고 낮에 졸리다고 호소하는 소아 청소년들도 있습니다.

때로는 잘 먹지도 않습니다.

과다 활동 장애 및또는 주의력 결핍장애도 생길 수 있습니다.

The more information you know about your child’s age, gender, past medical history, family history, examination findings, and clinical tests, the more helpful it is to give an answer. We will give you an answer based on the information you provided.

Q. When you catch a cold from a young age, it is difficult to breathe.

A.

A cold is a viral upper respiratory tract infection caused by infection of the mucous membrane of the upper respiratory tract, such as the nasal cavity and pharynx. When you have a cold, your nose can get stuffy, making it a bit difficult to breathe. However, it is normal to not have a stuffy nose with a cold that makes breathing that hard. Colds and allergic rhinitis, colds and bronchial asthma, especially night sleep-induced bronchial asthma, or colds, allergic rhinitis, and bronchial asthma may coexist. This can cause severe breathing difficulties. In addition, an enlarged adenoid can make breathing worse.

Q.

I also have a bit of snoring as usual.

A. Most children and adolescents with snoring have allergic rhinitis, which can cause swelling of the nasal mucosa. As a result, the nasal airway area may become narrower. The adenoids and tonsils may become enlarged, which in turn narrows the passageway between the pharyngeal and nasal passages and the tonsils, making the portion of the pharyngeal airway narrower and eventually narrowing the entire upper airway passageway. If the adenoids become very enlarged and tonsils are enlarged, it can make it difficult to suck air through the nasal airways into the airways, which can eventually lead to shortness of breath. Occasionally, you may develop obstructive apnea during sleep.

A CT scan of your sinuses, nasal passages, and adenoids can tell you how large your adenoids are and whether or not you have sinusitis.

Many children and adolescents with enlarged adenoids and allergic rhinitis also have sinusitis. After actively treating allergic rhinitis and treating sinusitis with appropriate antibiotics for 2 to 6 weeks, if the adenoids continue to enlarge, making breathing difficult or obstructive sleep apnea persists, consult an otolaryngologist and have the adenoid resection if he agrees. Please do surgery.

A. There are many children and adolescents who suffer from these health problems. This is because respiratory diseases such as colds and bronchial asthma are more likely to occur during the changing seasons. It can be treated with Cromolyn Sodium anti-inflammatory, which is a long-term (weeks to months) spray into the nasal passages. It has few side effects and is effective in preventing allergic rhinitis.

Cromolyn is an over-the-counter (OTC) drug in the United States that can be used at least once for the treatment of allergic rhinitis. In addition, there are corticosteroid sprays to treat allergic rhinitis by spraying into the nasal passages, and oral second-generation antihistamines such as Claritin are used to treat allergic rhinitis (see Table 2).

Repositioning a child’s head or neck while snoring may add less or more blockage to the part of the upper airway that is temporarily blocked, which may result in less, louder, or no snoring.
